Pills containing estradiol valerate and dienogest are a type of combined oral contraceptive (COC).

In some countries, COCs containing dienogest may be described as “fourth-generation” pills. However, this classification is not used consistently worldwide. The specific hormones and doses, along with individual health conditions and risk factors, are more important when choosing a contraceptive.

How is this pill different from other COCs?

One notable feature is that it contains estradiol valerate rather than the ethinyl estradiol used in many other COCs. Its progestin component is dienogest.

Can it be used for reasons other than contraception?

COCs can make bleeding patterns more predictable and may reduce menstrual bleeding and period pain. In some countries, the estradiol valerate/dienogest combination is also used to treat heavy menstrual bleeding in women who choose oral contraception.

However, birth control pills do not treat irregular ovulation to improve the chance of pregnancy. Because they suppress ovulation while being taken, people who are currently trying to conceive may need a different evaluation or treatment.

How effective is it for contraception?

Like other COCs, it is highly effective when taken correctly and consistently. Some products use different hormone doses at different points in the pack, so it is important to take the tablets in the specified order and follow the instructions for that product.

What about blood clot risk?

Estrogen-containing combined hormonal contraceptives can increase the risk of venous thromboembolism (VTE) compared with non-use. Evidence directly comparing estradiol valerate/dienogest with other COCs remains limited, so individual health and risk factors should be considered when choosing a contraceptive.
